Notification Form

• This form is to be submitted by the Training Provider.

• This form is for Skills Development course notifications only.

• Before submitting this form please ensure that the beneficiary has a valid training agreement for the course the notification is intended for.

Please Note: Notifications must be made at least 28 days before the start date. Any submitted with less than 28 days between the notification date and the start date will be returned and funding is not secured.

We may accept notifications at shorter notice, but you will need to get our agreement in writing (email) before sending in the notification.

All Notifications for courses within the current funding scheme (courses completed on or before 31 December 2023), must be received by 5pm on Monday 4 December 2023.

·    All Training and assessments must be completed by 31 December 2023

·    All claim invoices must be submitted by 12 February 2024
